About this program

Learn how to identify, analyze, and prevent claim denials before they cost revenue. This track covers denial categorization, root-cause analysis, and the appeals process — a specialty in high demand across hospitals and billing companies.
Every prevented denial is revenue the hospital never has to fight to recover.

About this course in detail

A denial is when an insurance company refuses to pay all or part of a claim. Every hospital and billing company deals with them, and each denied claim means delayed or lost revenue and extra work. Denials management is the discipline of understanding why claims are denied, correcting and appealing the ones that can be recovered, and, most valuable of all, changing processes so the same denials stop happening.

This program starts with the full life of a claim, from registration and eligibility checks to coding, submission and payment posting, so that you can see where problems enter. You then learn to read the payer's remittance, including the reason and remark codes that explain each adjustment, and to sort denials into types such as coding errors, eligibility problems, missing authorisation, duplicate claims and timely filing.

The second half is practical: finding root causes with simple analytical tools, writing effective appeal letters, designing prevention steps, and working through realistic claim scenarios. The skills are useful across hospitals, physician groups and revenue cycle companies.

Module-by-module guide

Revenue Cycle and Claims Lifecycle Overview

You trace a claim from patient registration through eligibility verification, coding, charge entry, submission through a clearinghouse, payment posting and accounts receivable follow-up. You learn the key measures teams track, such as denial rate, first-pass acceptance and days in accounts receivable, and what electronic claim and remittance files contain.

Denial Types: Coding, Eligibility and Authorization

Here you learn to read the remittance and understand the codes payers use to explain adjustments. You study the main denial families, including coding-related denials, eligibility and registration errors, missing prior authorisation, duplicate claims and timely filing, and learn which are preventable and which are not.

Root-Cause Analysis Techniques

Fixing one claim helps once, while fixing the cause helps for good. You learn to keep asking why until you reach the real cause, to use Pareto analysis to find the few reasons behind most denials, and to build simple trend reports by payer, code and department. You turn what you find into a short, prioritised action list.

Payer Appeal Letter Writing

You learn the levels of appeal and why deadlines matter, how a strong appeal letter is structured, which supporting documents to attach, and how to rebut a medical-necessity denial. You practise drafting appeals from templates and learn to track each one through to its outcome.

Denial Prevention and Process Fixes

Prevention starts at the front desk. You learn eligibility and registration checks, coding edits and claim scrubbers, pre-authorisation workflows, and how to feed lessons back to coders and billers. You also learn to measure whether a fix has actually reduced denials.

Real Claim Case Studies

You work through realistic denied-claim scenarios from the first look to final resolution, deciding whether to correct and resubmit, appeal or write off, and present a short root-cause summary of what you found.

How the training works

Each session combines explanation with case work. You are given sets of denied claims and asked to categorise them, decide the next step and justify it, which builds the habit of thinking like a denials specialist. Analytical exercises use spreadsheets so you learn to summarise denial data in a way that managers can act on.

Appeal writing is practised in class and reviewed individually, because clear, specific writing is what wins appeals. Reference lists of reason and remark codes, appeal templates and checklists are provided, and sessions are recorded for revision. The course closes with a case study you analyse end to end.

Tools you will use

  • Spreadsheets with sorting, filters and pivot tables
  • Reason and remark code lists
  • Appeal letter templates and tracking sheets
