Master Evaluation & Management Coding
Speciality Training • Real Case Studies • Certified Faculty
About this program
E/M coding accuracy is one of the most audited — and most valued — skills in medical coding.
About this course in detail
Evaluation and Management (E/M) services are the visits at the centre of most medical practices: a patient sees a doctor in the office, in hospital or in the emergency department, and the provider documents the encounter. E/M coding decides which of several visit levels was delivered, and because these visits are so frequent, small mistakes in level selection add up to large payment errors, which is why payers audit them closely.
The rules changed significantly in recent years. Office and outpatient visits were revised from 2021, and other E/M categories such as hospital, emergency and nursing-facility visits followed in 2023. Level selection now rests on either the amount of medical decision making involved or the total time spent on the date of the encounter, instead of the old emphasis on counting history and examination elements.
This focused program teaches those rules clearly and then applies them to chart after chart, so that you can choose levels confidently and defend your choice in an audit. It is an ideal next step after core medical coding training, and it strengthens one of the most heavily tested areas of the CPC exam.

Module-by-module guide
E/M Guideline Fundamentals (2021 Onwards)
You start with why E/M coding matters and what changed. You learn the five levels of an office or outpatient visit, how new and established patients are treated differently, and how the later updates extended similar logic to hospital and other settings. You also see how history and examination are still documented, but no longer drive the level on their own.
Medical Decision Making (MDM) Framework
MDM has three elements: the problems addressed, the data reviewed and analysed, and the risk of complications or of the patient's management. A level is supported when two of the three elements meet it. You learn what counts in each element, with worked examples for every level, so that scoring becomes a method rather than a guess.
Time-Based Coding Rules
Sometimes it is easier to choose the level by time. You learn when this is allowed, what activities count towards total time on the date of the encounter, how time must be documented, and how prolonged services are handled.
New versus Established Patient Visit Coding
Whether a patient is new or established depends on whether they have received professional services from the same provider group in the past three years. This module also explains consultations compared with referrals, emergency department and observation visits, initial and subsequent hospital care, and the basics of critical care and telehealth.
Payer-Specific Audit Patterns
You study where payers most often find errors: over-coding, under-coding, copied documentation and misuse of modifier 25. You see how auditors score E/M encounters and learn habits that keep your coding safe in an audit.
Real Chart Practice and Review
The final module is practice. You select levels on a range of realistic charts, score MDM and time side by side, and compare your reasoning with an auditor's. A timed practice set closes the course.

Career support
Resume review, mock interviews and guidance on openings, so you finish ready to apply.
How the training works
The course is short and intensive. Each session explains one part of the guidelines, shows it applied to a sample chart, and then gives you charts to code yourself. Because accuracy depends on reading documentation closely, a good part of class time goes to discussing why a level is or is not supported.
You receive reference tables, including an MDM scoring grid and quick guides for time-based coding, and recordings for revision. Assignments are reviewed with your mentor, who explains the logic behind each answer. The last module is entirely practice and finishes with a timed set.
Prior knowledge
Basic medical coding knowledge helps, because the course assumes you already know how to read a chart and use CPT. If you have not trained in coding before, speak to our counsellor about starting with the core program first.
Who is this course for?
Is this course right for you?
This program suits certified or trained medical coders who want to specialise, candidates preparing for the CPC exam who want to strengthen E/M, healthcare graduates who have completed core coding training, and coding auditors who need to defend level selection.
If you are new to coding altogether, the core Medical Coding program is the better starting point, and E/M works best as the follow-up. If you already code outpatient or physician charts and sometimes feel unsure about choosing between levels, this course targets exactly that gap.
You will get the most from it if you bring real questions and examples from your own work or study.
Roles this skill supports
Roles grow with experience, and many are open to freshers who have practised well.
E/M Coder
Specialise in office, hospital and emergency visit coding.
Professional-Fee Coder
Code physician services across clinics and hospital groups.
Outpatient Coder
Handle high volumes of outpatient encounters with confidence.
Coding Auditor
Review E/M claims for accuracy, a role employers value highly.
Compliance Analyst
Help organisations prevent coding risk before payers find it.
Certification guidance
Exam registration and fees are handled directly with the certifying body.
AAPC CPC
E/M is one of the most heavily tested areas of the CPC exam. This course strengthens exactly that part.
Careers, certification and what to expect
Strong E/M skills support roles such as E/M coder, professional-fee coder, outpatient coder and, with experience, coding auditor or compliance analyst. Auditing roles in particular value people who can explain why a visit level is correct. Specialist skills can also make you more productive and trusted within a coding team.
Certification
E/M is a large part of the AAPC CPC exam, so this course directly supports your preparation. The exam is held by the certifying body, and registration and fees are handled directly with them.
What to expect
Guidelines are updated, so good coders keep reading the yearly changes. Treat this course as a solid foundation: your mentor will point out where updates may affect your work, and you can return to the recordings and reference tables whenever you need a refresher.
